Articular cartilage reconstruction by "Autrologe Chondrozyten-Transplantation" (ACT)
While in classical measures a recent operation is necessary very often, the articular cartilage reconstruction is lastingly successful by the biomedical method of the autologous chondrozyte transplantation. How does this method work? Cartilage tissue is arthroskopic taken. In a special laboratory cartilage cells are millionfold increased, in order to transplant it into the defective cartilage. The Chondrozytes, the cartilage cells, are the only living components of a cartilage.
Approx. 30 million cells are bred in the IGOR (Institut for fabric and organ reconstruction) for each Transplantation. By the production of a very high number at cells within shortest time cells are faster available and freshly transplantable. From it a high Vitality of the cells results.
Conclusio: The more freshly and high-quality the cells are, which are embedded, so much the better are also cartilage production.
The technology by ACT
1 Knorpelbiopsat withdrawal
Arthroskopic inferred at a healthy, little loaded place of the joint, at least two about lens-large pieces of cartilage.
2 Cell culture breed
Within three weeks approx. 30 million patient-own cells are bred.
3 Taxidermy and Transplantation
Step 1:
Preparation of the defect. Debride the defective tissue carefully , in the healthy tissue a good cartilage shoulder shall arise.
Step 2:
Cut the dry fleece. The fleece not occupied yet with cells is cut accurately from edge of defect.
Step 3:
Manufacture the Fibringels. Mix particularly the modification solution manufactured by Fibrin with for each application.
Step 4:
Manufacture cell Fibringemi. All approx. 30 million fresh cells are carefully mixed with the modified Fibringel.
Step 5:
Fleece fill. The cut fleece from both sides with the entire cell Fibringemisch - approx. 30 million cells - fill, until it is completely impregnated.
Step 6:
Bring into the defect. The transplant impregnated with the cell Fibringemisch into the defect bring. With light pressure the Konstrukt in its thickness of the form and depth of the defect as well as the strength of the surrounding cartilage can be adapted. With very deep defects also a second situation can be applied. From the addition of the Thrombins results a ultrafeines, very sturdy hydraulic gel, which encloses, protects the cells optimally and ensures that the Konstrukt solidifies itself accurately after the form of the defect. A connection of old and new fabric develops.
Who is applicable for the Autologe Chondrozyten Transplantation (ACT)?
At present the method is used for Knoprpelschäden after injuries of the knee joint with physiologically younger patients (18 - 55 years). Particularly damage to the joint surfaces of the thigh bone and the kneecap is suitable for this innovative treatment technology. Beyond that the method is suitable still for application at other joints, whereby for each individual case it must be decided whether this technology tissue of the engineering is appropriate.
